Un rire qui se perd sur sa bouche

I've got an image. An elderly lady in a nightdress. Hair is in disarray, mouth is ringed with black. Teeth are black, and somewhat broken. I think the mouth has something to do with cause of death. But she can't die yet (even though she's already dead), because she can't relax her mouth. She is angry. But also, she doesn't want to hurt. But she wants the pain to stop. She resents anyone who approaches her and can't make the pain stop.

Explanations?

Too easy. She's fully cognizant, but doesn't understand that her world is fifty years in the past. Think ghost, not zombie.

Hmmm.... COULD be a comedy. But "old lady with mouth full of rocks" doesn't really have that knee-slapping quality.
